Please fill in both pages of the form. They can be sent back to us electronically, in this case without signature, or by mail in hard copies, please sign the summary in this event, together with the rest of the documentation requested.
Please send us the full or, after having handed over to us the forms electronically, the rest of the documentation by mail, incoming at the latest on January 15, April 15, July 15 or October 15 respectively (cf. first form, 6.).
Please provide proof of your latest qualifications (MD- or Ph.D.-thesis or diploma/MS; photocopies will suffice).
We will acknowledge receipt of your application immediately after expiry of the respective deadline.
